What service does EIM provide as the Home Care Coordination Agency?
- Arranges for the delivery of services to the long term care consumer
- Discusses assessment care plan, giving the consumer service options
- Identifies preferred service delivery pattern by the consumer
- Offers choices of service provider to the consumer
- Contacts provider of choice for availability of service
- Issues service orders to providers giving them authorization to provide services with specific instructions and preferences
- Manages an electronic information system for processing bills and statewide data
- Enters bills from providers
- Qualifies each line item through the system
- Successful lines items are electronically billed to the State
- Unsuccessful lines items are returned to the provider for correction
- Processes remittances
- Issues pay orders to the fiscal office for payment to providers
- Conducts activities on behalf of the consumer
- Advocates for Medicare services
- Finds a local service provider and enters into a contract with them
- Makes referrals for other services not covered by EIM
- Advocates for the most appropriate program placement
- Works with providers to develop new services to meet consumer's needs
- Assures that the services are least restrictive and cost effective
- Provides the lowest level of service that meets the consumer's needs
- Gives the consumer choices of service frequency
- Negotiates with providers for the lowest possible rates
- Considers alternative service delivery options (Ex: Home delivered meals rather than a PSS (formerly PCA) to prepare them)
