How can a person access Long Term Care services?
Long Term Care is provided through various programs and the services authorized under each program depend upon an individual's needs and financial status. Services can only be accessed after an individual is determined eligible. There are two ways to initiate eligibility:
- Go to the Department of Health and Human Services office nearest you and apply for Long Term Care services under Medicaid.
- Contact Goold Health Systems (GHS) at 1-800-609-7893 and ask for a Long Term Care Assessment.
Either method will most likely result in a nurse coming to your home and completing an eligibility assessment. The nurse assessor will inform you, at the time of the assessment, which programs and what services you are eligible for.
If you are found eligible, and choose to receive services under one of the programs EIM administers, your assessment will be forwarded to EIM. Other Long Term Care programs exist that are managed by agencies other than Elder Independence of Maine. At the time of your assessment, the nurse assessor will instruct you on how to get in contact with the appropriate agency to set up care.
EIM is unable to take referrals for services without an eligibility assessment from Goold Health Systems. |
