What's New in Long Term Care?
Your Elder Independence of Maine Identification Card
Since December of 2000, Elder Independence of Maine has been sending all active consumers Identification Cards that have your name, address, the program that you are receiving services through and your dates of eligibility.
All consumers will receive a new card at the time that their services begin and after each reassessment. The cards are printed daily so you can expect to receive your card within a week of your assessment.
Please keep your card handy. The identification card is for you to present to your home health providers, health care providers and to the Goold Nurse who completes your re-assessment. The card is a way to identify that Elder Independence of Maine is assisting you in coordinating your home health care. The cards do not pay for prescriptions or medical appointments.
