What's New in Long Term Care?
Updated Plan of Care Letter
The purpose of this letter is to outline the most current plan of care that you are receiving. Each time your POC is changed or renewed, Elder Independence of Maine will send you an Updated Plan of Care letter. We also send a copy to your primary care physician so that she/he is kept informed of the home healthcare services you receive. If you receive a letter that does not correctly describe the services you receive, please contact your Care Manager. |
