Are you interested in the Family Provider Service Option (FPSO)?


Thank you for your interest in the FPSO! Unlike being eligible for a state-funded program, the FPSO is about being an Employer, with the responsibilities that go along with having an employee(s).

To become an FPSO Employer:

  • An employer must be an EIM consumer, or a consumer’s family member.
  • An Employer must be an adult at least 21 years of age.
  • A potential Employer will have a criminal background check completed by EIM.
  • The potential Employer would register as a PCA agency with the State Division of Licensing. The cost is $25.00.
  • Fill out paperwork with a “Fiscal Intermediary” who works on the Employer’s behalf to do the paychecks and taxes for the employee(s).
  • Obtain workers’ compensation insurance for the employee(s). Premium is paid for by the program funding source.
  • Attend a training and sign an Agreement with EIM to follow all program rules and regulations.
Ongoing Responsibilities of an Employer:

  • Find, employ and direct personal care employee(s).
  • Work with the Fiscal Intermediary if any payroll issues arise.
  • Decide if an employee can be hired following background checks.
  • Communicate with the EIM Care Manager who manages the consumer’s overall Plan of Care and authorizes the services to be provided.
  • Keep consumer and personnel records.
  • Maintain workers’ compensation insurance.
Although there are several steps to complete, there are people available here at EIM to help you through the process. To assist you with your decision, we want you to be aware of the above information. We have found that those who have taken the steps and qualified, are glad to have done so. Agency staffing may also be available to the consumer while going through this process.
